# Does PRP for Achilles tendonitis help?

Does PRP for Achilles tendonitis help when the back of your ankle stays sore? Good studies haven’t found extra help for soreness that had lasted for months. PRP means platelet-rich plasma. Staff spin blood taken from you to collect more platelets in one layer, and a medical provider returns that layer as a shot near the tendon. The Achilles joins your calf to your heel and helps you push off while walking.

## When does Achilles soreness need urgent care?

Soreness that builds slowly may feel stiff when a walk starts and ease after a few minutes. A rupture means the tendon has torn suddenly, often with a pop, weak push-off, or trouble standing on your toes. If that happens, it’s time to go to urgent care or an emergency room. Fever, spreading redness, new numbness, or a joint that is hot and swollen also needs prompt medical care. These problems shouldn’t wait for a routine clinic visit.

A sudden loss of strength matters more than the PRP question.

## What may help when the soreness came on slowly?

You might reduce hills, fast walking, or running for a while, without staying still for weeks. A physical therapist can show you slow heel-lowering exercises and help you add more only as the tendon settles. QC Kinetix offers a consultation and regenerative treatments, which are non-surgical choices it makes with your blood at the clinic. Yet one study found that PRP didn’t help more when both groups did the same heel exercises and one group got a salt-water shot. In another trial, PRP didn’t improve the result compared with a dummy needle procedure.

For this tendon, the findings don’t support much hope from PRP.

1. In a double-blind, placebo-controlled trial of 54 patients with chronic midportion Achilles tendinopathy, all doing eccentric exercises, VISA-A scores improved 21.7 points with PRP and 20.5 points with saline over 24 weeks - an adjusted between-group difference of -0.9 (95% CI -12.4 to 10.6), excluding the predefined relevant difference of 12 points. PRP added nothing to eccentric loading.

2. A 24-site, 240-patient participant-blinded randomized trial of a single intratendinous PRP injection versus a sham (subcutaneous dry needle) for chronic midportion Achilles tendinopathy found VISA-A scores of 54.4 versus 53.4 at six months (adjusted mean difference -2.7; 95% CI -8.8 to 3.3) against a 12-point minimal clinically important difference. The authors stated the findings do not support the use of this treatment for chronic midportion Achilles tendinopathy.

3. An updated meta-analysis of six randomized trials (422 patients) found no benefit of PRP over placebo for Achilles tendinopathy on VISA-A at 3 months (mean difference 1.7; 95% CI -1.8 to 5.2), 6 months (0.5; 95% CI -8.5 to 9.3) or 1 year (-7.9; 95% CI -27.3 to 11.6), nor on VAS pain at 3 months. Funnel-plot asymmetry suggested publication bias inflating apparent benefits. The authors wrote that PRP should not be used to treat Achilles tendinopathy until high-quality trials show a clear clinical benefit.

PATH-2 randomised 230 adults with acute Achilles tendon rupture managed non-surgically to PRP or a placebo dry-needle injection across 19 UK hospitals, with a central laboratory confirming the PRP was of good quality with the expected growth-factor content. At 24 weeks there was no detectable difference in muscle-tendon function (limb symmetry index 34.7% versus 38.5%; adjusted mean difference -3.9%; 95% CI -10.5% to 2.7%) or in any secondary outcome or adverse-event rate.

5. The devices used to spin PRP at the point of care are cleared by FDA as clinical centrifuges, product code JQC, a Class I device under 21 CFR 862.2050 - for example the Biomet GPS Platelet Separation Kit (K030555, cleared 2003) and the Harvest SmartPrep2 / SmartPrep Platelet Concentration System (K103340, cleared 2010). That clearance covers the equipment that separates blood. It is not an FDA approval of platelet-rich plasma as a treatment for osteoarthritis, tendinopathy or any other orthopedic condition, and copy must never blur the two.

## Would you like someone to look at the sore area?

At QC Kinetix, a doctor, nurse, or another trained medical provider can listen and examine the place that hurts. The non-surgical choices include PRP, which stands for platelet-rich plasma. Staff spin blood drawn from you, keep the layer with more platelets, and give it back as a shot near the sore area. They’ll explain whether it may fit and what the full cost covers.

The nearby office is at 1100 S. Dobson Rd., Suite 210 in Chandler. Call (602) 837-PAIN to confirm scheduling and plan your drive. If another kind of care makes more sense, you can ask what comes next.
